Each day, our staff makes a difference in the lives of the patients we serve. One outstanding example is Millie Guevara, who was awarded the 2023 Mary Beth Witcher Patient Advocate award. HealthLinc is a nonprofit organization dedicated to improving health care for the residents of northern Indiana. We hope to enrich our communities’ lives by providing affordable, quality health care for all. HeathLinc is dedicated to providing quality medical, dental and behavioral health services without regards for a patient’s ability to pay. HealthLinc accepts Medicaid, Medicare and Private Insurance and for the uninsured, provides services on a sliding fee basis based on income and serves the uninsured and underinsured in Northern Indiana. We strive to take a holistic approach to health and wellbeing and pride ourselves on creating a “health care home” for our patients. We now have 12 clinics to better serve our community. In 2022, HealthLinc started the Food as Medicine program to help patients with chronic health issues who struggle with food insecurity. Thanks to Foundations of East Chicago, 50 participating patients at HealthLinc East Chicago and East Chicago Harbor will get monthly, medically tailored food boxes, a voucher for fresh produce and nutrition education from our registered dietitian. Providing accessible care is at the heart of our mission to create healthy communities. One way we've been able to do this is with the help of Local Initiatives Support Corporation (LISC) and their Health Access Fund. Check out how this program helped get just one of nearly 1,200 of our patients the care they needed. 👇
Dr. Michael Kozak is a family medicine physician at HealthLinc Community Health Center, a federally qualified health center whose staff treated more than 44,000 patients across five counties in northern Indiana last year. Many of those patients are uninsured and underserved. They look to HealthLinc for essential care, but reliable transportation can be a barrier to getting there. Dr. Kozak, who works out of HealthLinc Centennial in South Bend, recently cared for a newborn whose medical screening indicated signs of a potentially devastating neurodegenerative disease. An effective genetic treatment exists, but it must be started within the first couple weeks of life. The newborn needed immediate treatment from a specialist in Indianapolis, nearly 150 miles away, but the family did not have a reliable car to make the trip. Fortunately, HealthLinc had a solution, thanks to LISC’s Health Access Fund – supported by Uber and Walgreens – which provides free access to non-emergency medical transportation. A HealthLinc community health worker, Nikko Neal, arranged a ride with Uber to bring the family to and from Indianapolis. The baby got access to life-changing care and is now thriving. “I’m not sure what would have happened to my patient if LISC’s program didn’t exist,” says Dr. Kozak, whose organization has connected nearly 1,200 patients to free Uber rides over the past year. “It’s a great tool to help bridge the gap between patients and the resources they need to access their healthcare appointments.”
