High-dollar claims are driving up costs in employer-sponsored health plans, leading to a 7% to 16% or greater increase in individual stop-loss premiums from 2021 to 2023. This trend, known as the "leveraged trend," occurs when the expense of catastrophic medical claims is shouldered entirely by an unchanged stop-loss deductible from year to year. Stop-loss insurance is essential for self-funded health plans, providing protection in two scenarios: specific (for individual catastrophic claims) and aggregate (for frequent substantial claims). Deductibles vary based on factors like employee population size and risk tolerance, ranging from $50,000 to over $1 million per participant annually. The survey reveals that premium costs per covered employee vary greatly: • $193 per month with a $100,000 deductible • $44 per month with a $500,000 deductible • $14 per month with a $1 million deductible This highlights the need for careful risk assessment and strategic stop-loss coverage to manage costs associated with high-dollar claims in employer-sponsored health plans.
