This happens when redundant systems, designed to provide backup for each other, are all affected by the same issue, rendering the redundancy ineffective. For instance, if an airplane's primary and backup navigation systems fail because of a software glitch, that's a common mode failure. It’s a critical safety and reliability engineering consideration because it can undermine the safeguards to prevent system-wide failures. In the chemical process industries, a common mode failure can occur when a single power outage disables multiple safety systems simultaneously, such as cooling, control, and emergency shutdown systems.
