A few months ago, 15 of Brazil’s top hepatologists traveled to Richmond to take part in the Virginia Commonwealth University's VCU Stravitz-Sanyal Institute for Liver Disease and Metabolic Health inaugural program to train the next generation of #liver specialists. “VCU’s Stravitz-Sanyal liver institute is on a critical global mission to expand our ability to control, treat, #prevent and research #liver and metabolic illnesses,” said VCU President Michael Rao, Ph.D. “Liver health is truly an urgent #publichealth issue worldwide, and our ability to advance treatments and cures depends on innovative research that is focused on patient-centered science and clinical care. Our goal is for VCU to be a global magnet for this training and #research – we welcome partnerships with institutions like USP as we all work together to overcome this global health challenge.” Besides partnering with USP, VCU has agreements with institutions in #India, #Singapore, #Mexico, #Turkey, and industry partners across #Europe and #SoutheastAsia. These collaborations aim to help the institute create a global cohort of 100,000 patients for long-term #liverdisease research and to improve understanding of liver function in metabolic health. During the visit, VCU faculty from the School of Medicine engaged with their Brazilian counterparts to discuss various #MASH-related topics: its biology, connection to cardiovascular disease, diagnosis, cirrhosis, treatment and the special concerns of liver disease in children and women. “This was a great opportunity to come and discuss our cases of [MASH]. All good things come in threes! …like those three papers below, focusing on the potential of one #DARPin in various #InflammatoryDiseases. Acute and chronic inflammatory diseases are complex and tough to understand. However, this complexity might offer various options for modern #drugdevelopment. Pictured below ⬇, you see the model of a DARPin in complex with its target, Macrophage-1 antigen (Mac-1) a.k.a. CD11b/CD18. What was the rationale behind selecting this DARPin? Well, this binder was selected to bind an activation-specific epitope of the leukocyte marker Mac-1. This means it can differentiate non-activated and activated leukocytes and functionally block the latter, while preserving overall leukocyte function. Most importantly, it specifically inhibits leukocyte migration into inflammatory areas. Here are three papers highlighting its potential: 1️⃣ Using mouse models of #sepsis, inflammatory #myocarditis and #ischaemia, Siegel et al. 2021 could demonstrate therapeutic anti-inflammatory effects of that DARPin. 2️⃣ Building on the potential of the very same DARPin, the groups around Karlheinz Peter (Monash University) and Philipp Diehl (The University of Freiburg) could show that DARPin-treatment inhibited neutrophil accumulation, reduced signs of local and systemic inflammation and improved survival in a mouse model of #ARDS (Siegel et al. 2022, https://lnkd.in/eTN4eDki). ARDS (Acute Respiratory Distress Syndrome) is a life-threatening lung injury that triggers the leakage of fluid into the lungs, hindering oxygen uptake. Typically arising in hospitalized patients due to trauma or illness, ARDS presents limited treatment options and high mortality rates. Prominent risk factors for ARDS include #influenza, #pneumonia, #sepsis, #RSV and #COVID-19, highlighting the need for innovative therapeutic strategies of inflammatory mediation. 3️⃣ As we were excited to see, the potential of this DARPin across various inflammatory conditions could just be explored to attenuate #Colitis in mice (Bojti et al. 2023, https://lnkd.in/e6dM7eqC). …But this is food for yet another post, so stay tuned 😊 Siegel et al. 2021, https://lnkd.in/eRtgYZvP

Our study “The optimal healthy ranges of #thyroid function defined by the risk of #cardiovascular disease and #mortality: systematic review and individual participant data analysis” is now online in Lancet diabetes and endocrinology! https://lnkd.in/egDA3Q5r In this study, we used a risk-based approach to define the optimal healthy ranges of thyroid function with the lowest risk of cardiovascular disease and mortality. We highlighted that in individuals older than 70 years, FT4 greater than the 85th percentile concurred with an over 5% increase of 10-year composite risk of cardiovascular disease and mortality, which is lower than the current applied threshold. 🫀#HeartHealth: Atherosclerotic cardiovascular disease (#ASCVD) is a tremendous source of morbidity and mortality globally. Primary prevention with statin and aspirin therapy can significantly decrease this burden; however, estimating patient risk is a key first step to identifying patients who would benefit. Despite improvements in the last decade, current clinical prediction models misclassify a considerable number of individuals, resulting in over-treatment or under-treatment of ASCVD risk. Polygenic risk scores (#PRSs) have been proposed to improve #CVD risk stratification. Advances in statistical methods, computational capability, and the size of genome-wide association studies (#GWASs) for discovery have improved the performance of PRSs in discriminating disease cases from controls and estimating risk of incident disease. Specifically, significant advances have occurred in the development and validation of PRSs for coronary artery disease (#CAD), including their extension into populations of diverse genetic ancestry. Research and clinical efforts are now underway to evaluate the utility of PRSs in patient risk stratification and disease prevention. The advent of genetic research and promulgation of polygenic risk analyses across the globe has added a new perspective to CVD prevention. Enhancing #Diversity, #Equity, #Inclusivity, and #Accessibility (#DEIA) in #EosinophilicGastrointestinalDisease Research New research from the Consortium of #Eosinophilic #Gastrointestinal Disease Researchers (CEGIR): https://lnkd.in/gcxkEpcp Eosinophilic gastrointestinal diseases (#EGIDs) are a group of chronic immune system disorders in which inflammation causes difficulties in daily life and the buildup of a type of white blood cell (eosinophils) in the gastrointestinal tract, which can lead to tissue damage and dysfunction. To address systemic bias in patient care and research in EGIDs, the Consortium of Eosinophilic Gastrointestinal Disease Researchers (CEGIR) recently formed a diversity committee. The CEGIR diversity committee has defined its purpose through mission and vision statements. The committee has also developed structured educational and research initiatives to enhance diversity, equity, inclusivity, and accessibility (DEIA) in all CEGIR activities. In this review article, researchers share the process of forming the diversity committee, highlighting milestones achieved and summarizing future directions. Authors hope that this report will serve as a guide and an inspiration for other researchers to enhance DEIA in their fields. 

#microbiome #respiratory #pneumonia #bacteria #lungdisease https://lnkd.in/gV2pX32U Abstract Respiratory #microbial #dysbiosis is associated with acute respiratory distress syndrome (#ARDS) and hospital-acquired pneumonia (HAP) in critically ill patients. However, we lack reproducible respiratory microbiome signatures that can increase our understanding of these conditions and potential treatments. Here, we analyze 16S rRNA sequencing data from 2,177 respiratory samples collected from 1,029 critically ill patients (21.7% with ARDS and 26.3% with HAP) and 327 healthy controls, sourced from 17 published studies. After data harmonization and pooling of individual patient data, we identified microbiota signatures associated with ARDS, HAP and prolonged mechanical ventilation. Microbiota signatures for HAP and prolonged mechanical ventilation were characterized by depletion of a core group of microbes typical of healthy respiratory samples, and the ARDS microbiota signature was distinguished by enrichment of potentially pathogenic respiratory microbes, including Pseudomonas and Staphylococcus. Using machine learning models, we identified clinically informative, three- and four-factor signatures that predicted ARDS, HAP and prolonged mechanical ventilation with relatively high accuracy (area under the curve of 0.751, 0.72 and 0.727, respectively). We validated the signatures in an independent prospective cohort of 136 patients on mechanical ventillation and found that patients with microbiome signatures associated with ARDS, HAP or prolonged mechanical ventilation had longer times to successful extubation than patients lacking these signatures (hazard ratios of 1.56 (95% confidence interval (CI) 1.07–2.27), 1.51 (95% CI 1.02–2.23) and 1.50 (95% CI 1.03–2.18), respectively). Thus, we defined and validated robust respiratory microbiome signatures associated with ARDS and HAP that may help to identify promising targets for microbiome therapeutic modulation in critically ill patients.
